• Enhancing dementia care at Cleveland VAMC by developing a multidisciplinary group to: establish a consensus diagnosis, develop and maintain registry, establish care pathways and develop caregiver education programs
• Implementing a tele-dementia clinic to evaluate and provide consultative service to Veterans and caregivers
• Screening and detection of geriatric syndromes and multidisciplinary care coordination through GERI-VET, an accredited level 1 program that identifies at-risk older patients in the ED
• Establishing Transitional Care Model, which is a hospital-to-home program targeting older vulnerable adults at risk for poor outcomes
